New Patient Forms & Office Policies

At Smile Town Langley, we want your children's first visit to our practice as easy as possible. To get everything off to a smooth start, take a few minutes to read through our office policies and fill out our New Patient form prior to the first appointment.

Parking & Accessibility

Ample free parking for SmileTown Dentistry patients is available on the south side of the building. There are parking meters, but no action is required unless you plan on staying longer than 3 hours.

Our entrance is located on 208 Street beside G & F Financial Bank.

Methods of Payment

At Smile Town Langley, we accept these payment options:

DEBIT  •  MASTERCARD  •  VISA

Insurance 

Our office will submit claims to your insurance company on your behalf. We accept payment directly from insurance companies, but ultimately, you are responsible for paying any amount still owing after your child's appointment. Each insurance plan has different yearly maximums, limits, rules, and fee guides.

Make sure that you thoroughly familiar with the details of your plan, because if the claim is denied, you will be responsible for the full amount. » Download Patient Insurance Form

Before we treat a new patient, we carefully review all the previous dental and medical history, so that we can provide the best service possible.

Before your child's first visit to Smile Town Dentistry, download and fill out our New Patient Form to help make this process go smoothly. You can also arrive a few minutes early for the appointment and fill out the form at our office.

Cancellations & Appointments

To book your child's appointment at Smile Town Dentistry, please get in touch via phone or email, any time!

Our office is open Monday to Friday, 9am - 5pm, and on select Saturdays. Please call our office for details.

We know that life can sometimes get busy, and that as a result, appointments need to be cancelled or rescheduled. If you must reschedule or cancel an appointment for any reason, we ask that you contact us to let us know no less that 1 business day (24 hours) before the appointment was due to take place.

When you call, we'll cancel your appointment, and if possible, reschedule it for a more convenient time.
